Preparing for PAT Testing, Risk Assessment forms

& Contract details between the Company and Client.

In order to make our visit as short as is possible, and to avoid touching items that are not needing testing, we would appreciate it if you can arrange, where possible, the following to be done if you employ our services.

1. Unplug all items,(except items need to remain conected, eg working Freezers, Fridges etc.)

2. If large white good items (Over 15kg) please have them placed so we can easily get to the plug.  This both saves you money and us time and is safer as no handling is needed by us.

3. Smaller items that can be moved easily and collected together, please do so, ideally in one room, or if a lot of items, as few a rooms as possible.     Alterntively, group items togther in the relevent room, again making sure they are unpluged.

4. Let us know if you are unable to do this, as we can do it, but need to allow the right amount of time to the appointment. We do charge for moving larger (15kg+) items at £30 per item.  

5. If we have to move the larger items, we cannot be held responsible for any damages to flooring that might occur when, for example, a washing machine is pulled out. From past experience, the floor covering can get buckled when pushing a machine in under worktop, and then rips on being pulled out, unless a lot of time and care is spent doing so.   Hence the higher amount that would be charged.

6. We will be wearing gloves and assuming that the property will be empty of any other people, or they will be in other parts of the property while we are testing. This is why we ask items to be gathered together.

7. We are unable to take cash, but can take cheques, debit cards, credit cards and Internet Banking transfer.

We have recently reviewed our working conditions and going forward from 1st Jan 2022 we will be carrying out a risk assessment of any customers property/properties before we start work. 

 

To help us in this matter, please can you fill in the details asked for below and overleaf before emailing them back to us.

The list below is not exhaustive, and we would appreciate any additional information that you feel might be needed added to the assessment. 

If we find anything that we consider a risk that we have not been previously notified of, then the testing would be postponed and the client would be contacted to re-arrange a new time to attend.

Contract for PAT Testing.

I/We agree to employ J & A Creed (Services) Ltd (hereafter known as the company) of 16 Tower Street, Golspie, Sutherland, KW10 6SB  to carry out PAT work on appliances that are owned/managed by me at the addresses to be supplied by me.

 

I/We agree to supply the full address including postcode and a list of the items to be tested.  In the event of not supplying such a list, the company will test all items at the address and bill accordingly.

 

I/We understand that if the appliance cannot be moved to gain access to the plug the item will be reported as untested, and the owners take responsibility for that appliance.

 

I/We understand that if an appliance is in use and cannot be switched off,(Computers etc) then it will be marked as visual inspection only.

 

I/We agree to pay the charges as shown and in a timely manner of not exceeding 14 days from date of invoice, unless otherwise agreed and signed by both parties.  I/We understand that until payment is made, the certificate will not be issued.  Statements are not sent.

In the event of payment not being received in the time specified or as agreed,  then the Company reserves the right to charge interest at 10% each subsequent month or part month until the whole total amount is cleared.

For the avoidence of doubt regarding the above line an example: Client owe us £100.00 if after 1 month it was not paid, would increase to £110 and the month after that £121 and so on till the bill is paid.

 In the event that three months has elapsed since the end of the month the work was carried out in, the Company reserves the right to start proceedings to recover the costs (without further notice) and all the additional costs will be passed to the client, we may ask the court if we can take other items to the value of or greater than the amount owed, in order to defray the costs of recovering the money.

 

Once agreed(by email,writing or verbally), either party can only cease this agreement at one months notice in writing.(Unless it is one of the fixed price contracts).  If you wish to cancel before work starts, then you have 14 days to do so, unless you have requested the work to start at once or within 14 days.

 

The Company undertakes to carry out a Portable Appliance Test in the approved manner on each appliance that is required under the IET 5th Edition of the current code of conduct.

 

The Company will not be held responsible for any damage to the property caused in gaining access to PAT Test an appliance.*

 

The Company will ensure that the tester has suitable ID with them at all times.

The Company will issue a report after testing, and only issue the certificate and receipt once the account has been settled in full.

The Company will issue up to a maximum of two certificates per address, one for the tenant and one for the landlord or their agent. Additional copies will be charged for at £10.00 per copy.

The Company will be fully insured against damage to clients property while carrying out their duties. Public Liabilty of £5M (This is not including in the * above.)

 

The Company reserve the right to amend the prices at any time or if a new code of conduct or Government legislation is introduced during the next 12 months. (For the sake of clarity, Government is either UK or Scottish Government.) [Fixed price contracts are the exception]

 

I/We agree to pay the charges as agreed and in a timely manner of not exceeding one month from date of invoice.  Due to high bank costs, our means of payment are: Internet banking, BACS payment (Credit card on site) or Cash.   Regretfully as our bank is now 52 miles from Golspie, we are unable to accept Cheques unless paid 14 days before we start work.
